Gluteoplasty

Surgery  for the  gluteal zone,  gluteoplasty, is carried out by placing implants, it is very fashionable nowadays in many countries, because it improves the general outline of the zone,  producing a few scars, which can be very easily concealed in the  intergluteal fold. It is a fact that both men and women look each other’s gluteal zone. And that nobody wants to hear that his or hers is ugly. But “Ugly” means many things: It can mean that a gluteal zone is  downfallen, shapeless, flaccid, flat, too fat or simply too big.  It sounds a little exaggerated, but it is not: Having a nice gluteal zone is a source of self esteem, and it can be compared with having nice clothes or using fine colognes, etc. One knows from work mates,  college mates, friends or neighbors that their critical eyes will go “there” at least once a day.

This procedure is indicated both for men and women. Techniques are the same in both cases,  but there are some aesthetic differences. Men want to enhance the outline of the major gluteal muscle,  so that buttocks look more muscular.

Women want to get a rounded buttock, and, therefore,  a liposuction and a lipofilling are very common, besides the implants.

Implant is made of cohesive silicone gel, prothesis is flatter and more resistant than mammary prothesis.  This way, gluteal zone implants stand forces and pressures due to the major gluteal muscle (walking, running, jumping,etc.)

Prothesis is located beneath the major gluteal muscle. It can’t be noticed from the outside .The incision is practiced in the fold between both buttocks. As down as possible, to conceal it. Surgery requires general anesthesia, so that the muscle is perfectly relaxed and surgeons are able to put the prothesis easily in place. There is usually no need for drainages.

Patients must be very helpful during this surgery:   They must stay in bed, upside down, at the hospital , for 24 hours after surgery. Afterwards, they may get up but  have to be extremely careful, for one week: They must not sit unless it is necessary and carefully protecting the implant zone.  For two weeks more,  patients must sleep upside down. Physical exercise must be avoided for 5-6 weeks. All this makes the scar  around the prothesis  stable and prevents the prothesis  from shifting to a wrong position, producing an assimetry.

Prothesis is not rejected nor encapsuled. Risks of infection are low, but  a careful hygiene  is required, until the stitches are removed. Doctors also prescribe anti-biotics  for ten days after surgery, to minimize risks.

Women usually get a lumbar zone, waist and sides liposuction, to reshape these zones and improve and increase  the curve formed with the newly shaped buttocks. The removed fat  is injected in the inferior zone, just above the gluteal fold, in order to round the buttock  there.  Prothesis fill very well the two upper thirds of the buttocks, but not the inferior one. Men require just a prothesis, but women usually require a lipofilling, besides the prothesis.

When surgeons are doing a typical hips, buttocks, thighs liposculpture, they usually use the removed fat  to inject it in  the buttock central zone, which needs more projection and height, in order to have a more upward and youthful appearance.

Patients must remember to avoid intramuscular injections at the gluteal zone, because the prothesis could be pierced.

Finally, it is important to remind that  injections of liquid silicone or some other material must never be used. The correct prothesis can be extracted at any time, in case of complications or by patients wish; but liquid silicone infiltrates all tissues, has high risk of complications and is impossible to extract.
